Mental Health & Substance Use Screening

The doctor will assess your mental health and check for any history of harmful substance use, since this may affect your eligibility.

Completion of Form I-693

Once your exam and lab results are complete, the civil surgeon fills out and seals your Form I-693 in an envelope. This sealed envelope must be submitted to USCIS. You’ll also get a copy for your records.

What are the necessary prerequisites?

You need the following things as a prerequisite:

  • TB or Tuberculosis blood test for ages two years and older.
  • Gonorrhea test for ages 18 and older.
  • Syphilis or RPR test for ages 18 and older.
  • You should have vaccinations for Measles, Mumps, Rubella, Tetanus, Diphtheria, Polio, Varicella, Hepatitis B (birth till 59 years of age), COVID 19 & Influenza (in the Flu season).
  • If you had chickenpox when you were a little kid, you do not need the varicella shot.
  • If your TB test is positive, you will need a Chest X-ray also.
